Advanced Practice Provider-Family Clinic (Healthcare)
Fall River Health Services is looking for an Advanced Practice Provider to provide familyfocused care to the Black Hills Community. In this role, you will be responsible for educating patients about disease prevention, performing diagnostic tests, and managing the overall care for a wide variety of patients. This role must be able to effectively diagnose and provide treatments for a wide range of patients' needs. Responsibilities: Responsible for providing quality primary clinical care (in consultation with the supervising/collaborating physician or independently. Depending on License). This includes working with clinic staff and other medical staff members to ensure clinical standards are maintained. Assess the health status of individuals of all ages by obtaining a database that includes a history, physical examination, investigative studies, psychosocial assessment, and development of a problem-oriented record. Provide counseling, education, and intervention for the promotion, maintenance, and restoration of optimal levels of health for disease prevention for individuals of all ages, families, and communities. Provide assessment and appropriate management of patients of all ages with common, acute undiagnosed problems in a manner consistent with protocols for each problem, which indicates the appropriate database as well as specific assessment and management plans for a variety of selected findings. The incumbent makes referrals as appropriate. Responsible for timely completion and adequate, complete documentation of patient records and is responsible for maintaining and respecting the confidentiality of patient information by medical bylaws, HIPAA, and PA/CNP rules and regulations. This position in collaboration with facility staff cooperates in all processes necessary to obtain timely payments. Assess and provide follow-up management of patients of all ages with common chronic health issues in a manner consistent with protocols for each condition as well as specific impressions and management plans for a variety of selected findings.Develop professional role identity as a primary care provider, recognizing professional limitations. Refers when needed, consults, and maintains a close working relationship with supervising/collaborating physicians and other members of the health care team.Responsible for maintaining positive relations with patients, all health care providers, co-workers, and members of the community. Core Competencies: Comfortable working independently and autonomously. Critical thinking and troubleshooting skills to assess and begin treatment for patients and/or refer to higher-level care. Excellent customer service and communication skills (including oral, written, and listening). Proficient knowledge of Hospital Consumer Assessment of Healthcare Providers (known as Hospital CAHPS) and ability to achieve top box scoring. Proficient and timely documentation and organizational skills. Self-motivated, adaptable, flexible, and able to make sound judgment calls. Flexible and willing to accept other duties as assigned.
