The Residential Care Manager ensures the efficient operation and management of a residential care facility, providing comprehensive support to residents. This role involves direct interaction with Members, their families, Authorized Representatives (ARs), caretakers, and other support persons. The Residential Care Manager collaborates with a multi-disciplinary care team to address gaps in care and coordinate services across various health domains, focusing on whole person wellness and recovery.
How will I contribute
Engage directly with Members, families, ARs, caretakers, and other support persons to provide high-quality care and support. Conduct outreach, education, coordination, health promotion, assessment, and care management. Develop and implement care plans targeting the most vulnerable persons in the community. Ensure the integration of services addressing social determinants of health (SDOH). Coordinate with healthcare and social service professionals to assist Members with funding, medical care, Veterans benefits, SSI, mental health, and other needs. Advocate on behalf of Members with healthcare professionals. Use motivational interviewing, trauma-informed care, and harm-reduction approaches. Ensure a seamless experience for Members and non-duplication of services. Offer services where the Member lives, seeks care, or finds most accessible within managed care plan (MCP) guidelines. Connect Members to other social services and supports, including transportation. Coordinate with hospital staff on discharge plans. Accompany Members to office visits as needed and according to MCP guidelines. Monitor treatment adherence, including medication. Provide health promotion and self-management training. Monitor vital signs if nursing certified. Assist in the day-to-day operations of the residential care facility. Ensure the facility is maintained in accordance with health and safety standards. Oversee the provision of ECM services, including assessment, care plan development, and implementation. Assess the needs of residents upon admission and throughout their stay. Engage eligible Members to promote whole person wellness and recovery. Focus on helping Members achieve appropriate housing, chronic and/or acute symptom recovery, and absence of involvement with the criminal justice system. Ensure compliance with all relevant regulations and standards of care to maintain the highest quality of service delivery. May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled). If such situations arise, staff will be required to call local emergency personnel to help deescalate the crisis, use crisis intervention techniques, and consult with supervisor as soon as practically possible. What I bring
High School Diploma (or Equivalent) 1 year experience working with one of the following target populations: homeless, severely mentally ill or substance use disorder diagnosed, and criminal justice. Class C Driving License Knowledge of behavioral health, criminal justice system, and housing/homelessness, including challenges, needs, resources, interventions, and/or other content knowledge. Able to work occasional evenings and weekends and work in inclement weather and in unsanitary conditions as needed. Ability to prepare and conduct care management assessments and plans, communicate effectively with groups and individuals both verbally and written, instruct and assist in obtaining services and information. Develop program material and use effectively, establish, and maintain working relationships with a variety of agencies, organizations, and individuals. Empathy and compassion, strong communication skills, collaborative nature, resilience and patience. Commitment to whole person wellness. Bachelor's degree in Healthcare Management, Nursing, or a related Preferred Proficiency in motivational interviewing, trauma-informed care, and harm-reduction approaches Preferred Knowledge of regulations and standards in residential care Preferred Bilingual English/Spanish preferred. Certified Nursing Assistant (CNA) or Medical Assistant (MA) experience desired. Persons with lived experience highly desired.
