School Based Care Coordinator (Hybrid, Morrisville, North Carolina Based) (R&D)
The School Based Care Coordinator is responsible for assisting individuals with complex chronic Mental Health, Substance Use, and/or Intellectual Developmental Disability diagnoses navigate the behavioral health, physical health, and school system. The coordinator shall provide a person-centered approach to identify service needs, link to the most effective level of care, use a system of care approach in collaboration with stakeholders, monitor quality of care/ use of evidence-based best practice, and ensure consistent collaborative approach with school system.
This position will require travel within Wake County and to the home office (Morrisville, NC) as needed for meetings. We anticipate a projected start date of July 8,2024

Minimum Requirements
A Master's degree in a Human Services field (such as Psychology, Counseling, Social Work, etc.) is required, and two (2) years of post-degree progressive experience providing similar services to the population served. Additionally, valid licensure or provisional license in profession (a LCSW, LCMHC, LMFT, or LCAS) is required.
Employee is responsible for complying with respective licensure board's continuing education/training requirements in order to maintain an active license.
Additional Training Preferred:
Experience in the public mental health field and school system is highly desired due to the complex work of the program.
Salary Range
$58,480 - $76,024/Annually
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
